Information for New Patients
I AM CURRENTLY ACCEPTING NEW PATIENTS.
What to expect:
After you schedule your first appointment, you will receive information in the mail. The cover letter included in packet has a checklist of things that you should bring to your first appointment, including you insurance card, if you plan to bill your insurance provider. The packet will also include information regarding my practice policies as well as The Patient and Family Information Form. Additional copies of these two forms can be found below.
Please note: if you have an insurance plan that requires a referral from your Primary Care Physician, and most plans do, please contact his or her office prior to our first appointment. It is very important that a referral be in place before our meeting, or you may be required to cover the total amount for our sessions.

My office is located in the East Tower Building. It is near the intersection of East Avenue and Clover. If you are traveling East on 490, take exit 22 (Penfield Road). Turn West on Penfield towards East Avenue. The East Tower Building will be in front of you when you must turn left or right. The entrance to the office and parking are in the back of the building. Please see Google Maps for an interactive map. Please check in at the front desk.
Medication
As a psychologist, I do not prescribe medication. If you are interested in hearing more about medication options or if I think medication might be helpful, I can assist you in obtaining a medical evaluation. I am also happy to consult with your pediatrician/ primary care provider.
